Membership & Class Pack Terms and Conditions
These Terms apply to FORME recurring memberships and non-recurring class packs. They should be read alongside the FORME Client Health Declaration, Participation Waiver, Privacy Notice and Studio Rules. By purchasing or using a FORME membership or class pack, you agree to the terms that apply to your purchase.
Product-specific information shown at checkout — including price, credits, activation date, expiry, minimum term and any promotion-specific rules — also forms part of the contract. If a product-specific term conflicts with these general Terms, the product-specific term will apply to that product, provided it is fair and was clearly disclosed before purchase.

In these Terms:
- “FORME”, “we”, “us” and “our” mean DSF Companies Limited trading as FORME Reformer Pilates.
- “Member”, “you” and “your” mean the person purchasing or using a membership or class pack.
- “Membership month” means the period beginning on the membership Activation Date or recurring renewal date and ending immediately before the corresponding date in the following month.
- “Class credit” means one booking entitlement for an eligible class.
- “Activation Date” means the date on which a membership or class pack becomes active under section 5.
2. Eligibility and personal use
Members must be at least 18 years old unless FORME has expressly agreed to accept a younger person with appropriate parent or guardian consent.
Memberships and class packs are personal to the named member and cannot be shared, transferred, resold or used to book classes for another person unless FORME expressly agrees otherwise in writing.
We may ask for reasonable proof of identity before allowing a membership or class pack to be used.
3. Health, pregnancy and suitability
Before attending a class, every member must complete and accept the FORME Client Health Declaration and Participation Waiver.
Members are responsible for:
- ensuring that they are medically and physically able to participate;
- disclosing relevant injuries, illnesses, pregnancy, recent surgery, medical conditions or medication;
- informing the instructor of any material change in health before participating;
- seeking medical advice where they are uncertain whether reformer Pilates is suitable; and
- exercising within their own capabilities and stopping if they experience pain, dizziness, unusual shortness of breath or other concerning symptoms.
Pregnant members must tell FORME and the instructor before participating. Where appropriate, FORME may recommend a modified class, request confirmation that participation is suitable, or decline participation where a class cannot be safely adapted. Members returning after childbirth, surgery or significant injury may also be asked to obtain medical clearance where reasonably appropriate.
FORME instructors are not acting as medical practitioners and cannot diagnose a condition or guarantee that exercise is medically suitable for a member. We may refuse or suspend participation where we reasonably believe that continuing could create a material safety risk to the member, instructor or another person.
4. Memberships and class packs
The benefits, price, number of class credits, minimum term, expiry period and any restrictions will be shown at the point of purchase.

Membership or class-pack purchase does not guarantee a place in a particular class, at a particular time or with a particular instructor. All bookings remain subject to availability.
5. Activation, billing dates and class-pack expiry
Recurring memberships. Unless the product description states otherwise, a recurring membership activates on the scheduled date of the member’s first booked class (the “Activation Date”), provided that first class is booked to take place within 30 days of purchase. The initial membership payment is taken at purchase. The recurring payment date and credit-renewal date are then calculated from the Activation Date. If no first class is booked to take place within 30 days of purchase, the membership will activate automatically on the 30th day after purchase.
Where the corresponding calendar date does not occur in a particular month, the recurring payment and credit-renewal date may fall on the final day of that month.
Class packs. BLOCK 5 and BLOCK 10 are non-recurring purchases. Unless the product description states otherwise, a class pack activates on the date of purchase and expires at the end of its stated validity period. FORME may agree to a different activation date or extension where this is expressly confirmed in writing.
The purchase confirmation will identify the product purchased, price, billing frequency (if recurring), minimum commitment, Activation Date or activation method, expiry rules, cancellation procedure and any applicable cooling-off rights.
6. Payments and recurring billing
Fees must be paid using a payment method accepted through the FORME booking system. Consumer prices will include VAT where VAT is applicable.
By purchasing a recurring membership, you authorise FORME and/or its payment provider to collect the stated recurring fee on each payment date until the membership ends in accordance with these Terms.
You are responsible for keeping payment details valid and current. Payment is not treated as made until it has been successfully received.
If a payment fails, FORME may:
- notify you and request an alternative payment;
- make a reasonable further attempt to collect the outstanding payment;
- temporarily suspend booking and attendance rights while payment remains outstanding; and
- cancel the membership where payment remains outstanding after reasonable notice.
Any administration charge relating to a failed payment must be reasonable and reflect the actual or reasonably estimated cost caused by the failed payment. FORME will not impose a disproportionate penalty.
7. Minimum membership term and automatic continuation
FORME 4, FORME 8 and FORME 12 have a 3-month minimum term unless a different minimum term is clearly stated before purchase.
During the minimum term, the member remains responsible for the agreed recurring payments unless:
- a statutory cancellation right applies;
- FORME accepts early termination under section 18;
- FORME materially breaches the membership contract; or
- continuing the contract would otherwise be unlawful or unfair.
At the end of the minimum term, the membership continues on a monthly rolling basis until cancelled in accordance with section 19. FORME will not require cancellation only during an artificially narrow cancellation window.
8. Cooling-off rights for distance and off-premises purchases
Where the law gives you a cancellation right because you purchased a membership or class pack online, by telephone or away from the studio, the statutory cooling-off period is generally 14 days beginning after the contract is made.
To cancel during the cooling-off period, send a clear cancellation request by email to hello@formereformer.co.uk or use any cancellation facility made available in your online account or on the FORME website. You do not need to use any particular wording.
If you expressly ask FORME to begin providing services during the cooling-off period and then cancel before the cooling-off period ends, FORME may deduct a proportionate amount for services supplied before cancellation where the law allows. Where a service has been fully performed during the cooling-off period, the right to cancel may be lost only where the legal requirements for that loss of right have been met.
Any refund due following a valid statutory cancellation will be made using the original payment method unless otherwise agreed, within the period required by law.
9. Booking classes
Membership does not automatically reserve classes. Members must book through the FORME booking system.
- Bookings are subject to availability.
- A booking must be within the class types and restrictions included in the member’s product.
- One confirmed booking uses one available class credit.
- Bookings are personal to the named member.
Members should not make speculative bookings that they do not reasonably intend to attend. FORME may introduce reasonable forward-booking or booking-volume limits where needed to protect fair access, provided any material restriction is clearly disclosed before purchase or introduced on reasonable notice and does not materially undermine the product purchased.
10. Class credits, expiry and rollover
Unless the product description states otherwise:
- membership credits become available on the relevant monthly renewal date;
- one eligible class uses one credit;
- credits must be used during the relevant membership month or class-pack validity period;
- unused credits do not automatically roll over;
- credits have no cash value;
- credits cannot be transferred to another member; and
- credits cannot be exchanged for private sessions, retail products or other services unless FORME expressly agrees.
FORME 12 rollover. Up to one unused FORME 12 credit may roll into the immediately following membership month, provided the membership remains active. A rolled credit expires at the end of that following membership month and cannot roll forward again.
Where FORME cancels a booked class, the associated credit will be restored or an appropriate alternative remedy provided. A restored credit will not be allowed to expire solely because of FORME’s cancellation before the member has had a reasonable opportunity to use it.
11. Class cancellation and no-shows
Members must cancel a class through the booking system at least 12 hours before its scheduled start time.
Where sufficient notice is given, the credit will be returned to the member’s account, subject to its original expiry date.
Where a member cancels after the deadline, does not attend, or arrives too late to participate safely, the class credit may be treated as used.
FORME may, at its reasonable discretion, restore a credit in exceptional circumstances such as a genuine emergency. This does not create an automatic entitlement to a credit or refund in every case.
12. Waiting lists
Where a member joins a waiting list:
- the booking system may automatically confirm a place when one becomes available, where that process is clearly stated;
- the normal cancellation policy applies once the booking is confirmed;
- the member is responsible for monitoring booking notifications; and
- FORME may set a reasonable cut-off time after which automatic promotion from a waiting list stops.
Members should remove themselves from a waiting list when they can no longer attend.
13. Arrival, grip socks and participation requirements
Members should arrive at least 5 minutes before class and approximately 10 minutes before their first session.
Suitable exercise clothing and grip socks are required for reformer classes unless FORME expressly states otherwise. FORME may refuse participation where clothing, footwear, hygiene or equipment requirements are not met and safe participation cannot reasonably be achieved.
For safety reasons, an instructor may refuse entry after a class has begun, particularly where the warm-up or safety briefing has been missed, entry would materially disrupt the class, or the member cannot be safely prepared on the reformer. A refused late arrival may be treated as a late cancellation and the credit may be used.
14. Changes to classes, timetable and instructors
FORME may make reasonable operational changes to instructors, timetable times, class names or formats, equipment, class capacity or temporary room arrangements.
The identity of a particular instructor is not guaranteed unless a service was expressly sold on that basis.
Teaching style, music, sequencing and class intensity may vary between instructors and sessions.
Where FORME cancels a class, the member will receive restoration of the relevant credit, an alternative class, or another fair remedy.
A temporary or minor timetable change will not normally entitle a member to cancel an entire membership.
Where changes are substantial, repeated or materially reduce the service originally purchased, the member may be entitled to end the membership without additional charge or receive another appropriate remedy.
15. Studio closures
FORME may occasionally close for public holidays, planned maintenance, staff training, essential repairs, health and safety reasons, or circumstances outside our reasonable control.
Planned closures forming part of the published timetable will not normally extend a membership where the membership price and expected timetable reasonably account for them.
Where an unplanned closure materially prevents use of a membership or class pack, FORME will provide a fair remedy according to the extent of the disruption, which may include extending credit expiry dates, extending the membership period, providing replacement classes, applying a proportionate account credit or refunding the affected part of the service where appropriate.
16. Booking-system and technical interruptions
FORME uses third-party technology and payment systems to support bookings, notifications and payments. Temporary internet, software or platform outages may occasionally prevent normal use.
FORME is not responsible for a failure caused solely by a third-party system or event outside its reasonable control, but will take reasonable steps to provide an alternative booking or communication route where practicable. A technical issue will not be used to deprive a member of a class credit or cancellation right where the member can reasonably show that the issue prevented timely action.
17. Freezing a recurring membership
A member may request a membership freeze by giving at least 14 days’ notice before the intended freeze start date. Standard freezes are subject to the following rules:
- minimum freeze: one full membership month;
- maximum standard freeze: one full membership month in any rolling 12-month period;
- permitted reasons include injury, illness, pregnancy, extended travel or another reasonable circumstance accepted by FORME;
- administration charge: none;
- reasonable supporting evidence may be requested where appropriate, particularly for a medical or extended freeze;
- billing and access are paused during the approved freeze;
- no class credits accrue while the membership is frozen;
- the minimum membership term and next billing date are extended by the length of the approved freeze; and
- freeze requests are not normally applied retrospectively unless FORME agrees in exceptional circumstances.
FORME may agree to a longer or additional freeze for pregnancy, serious illness, significant injury or other exceptional circumstances. Any medical information requested will be limited to what is reasonably necessary.
18. Ending a membership during the minimum term
A member may request early cancellation where a significant change in circumstances makes continued use unreasonable. FORME will consider such requests fairly and reasonably. Examples include:
- serious injury or illness that materially prevents participation;
- pregnancy-related medical restriction;
- relocation such that regular attendance is no longer reasonably practicable;
- redundancy or serious and unexpected financial hardship; or
- a material change to the services provided by FORME.
FORME may request reasonable supporting evidence. Where evidence is requested, it should be proportionate to the reason given and need not disclose unnecessary medical detail. Where early cancellation is accepted, it will normally take effect from the next scheduled payment date after approval. No further recurring payments will be collected after the effective cancellation date. This section does not limit any stronger statutory right the member may have.
19. Cancelling after the minimum term
After the minimum term, a member may cancel at any time by using any cancellation facility available in their online account or by emailing hello@formereformer.co.uk. Cancellation will take effect at the end of the current paid membership month unless the member has a statutory right to an earlier cancellation or FORME agrees otherwise. No new monthly fee will be collected after the effective cancellation date. FORME will confirm cancellation in writing or electronically and state the effective end date. Verbal notification to an instructor will not normally be sufficient unless FORME confirms acceptance in writing. The cancellation process will not be made materially more difficult than the process used to enter the recurring membership.
20. Refunds and unused credits
Except where required by law, expressly stated in a promotion, or otherwise agreed by FORME, membership fees and class-pack payments are not refundable merely because a member changes their mind after any applicable cooling-off period has expired or does not use all available credits. Unused or expired class credits do not create a cash entitlement. However, this does not affect:
- statutory cooling-off rights;
- rights relating to services not provided with reasonable care and skill;
- any remedy due because FORME materially fails to provide the purchased service;
- any fair remedy under sections 14 or 15; or
- any refund or credit that FORME agrees in exceptional circumstances.
Any refund due will normally be returned to the original payment method unless otherwise agreed or technically impracticable.
21. Price changes
FORME may change recurring membership fees after giving at least 30 days’ written or electronic notice. The notice will state the new price, the date it takes effect and the member’s right to cancel before the increase applies. A price increase will not be applied retrospectively. During a clearly stated fixed-price minimum term, the membership price will not normally change until that term ends unless the possibility and method of adjustment were clearly explained before purchase and the change is fair, proportionate and lawful.
22. Promotional, introductory and gifted products
Promotional and introductory offers may have eligibility requirements, stated start and expiry dates, usage limits, excluded classes or times, or a limit of one offer per person. FORME may cancel or reverse duplicate promotional purchases made contrary to clearly disclosed eligibility rules, with an appropriate refund of any amount not properly due.
Any automatic conversion from a promotional or trial product to a paid recurring membership must be clearly disclosed before sign-up, including the price and timing of the first recurring payment.
Gifted class packs or memberships remain subject to the product-specific expiry and usage rules. Unless the product is expressly described as transferable, the recipient must use the product personally once it has been allocated to them.
Promotions do not affect statutory consumer rights.
23. Studio Rules and member conduct
Members must comply with the FORME Studio Rules and must:
- treat instructors, staff and other clients respectfully;
- follow reasonable safety instructions;
- use equipment only as directed;
- avoid threatening, abusive, discriminatory or materially disruptive behaviour;
- not attend while impaired by alcohol or non-prescribed drugs;
- not photograph or record other people without permission;
- respect the privacy and personal space of other members;
- wear suitable clothing and required grip socks; and
- observe reasonable hygiene and equipment-cleaning requirements.
A serious or repeated breach of the Studio Rules may result in refusal of entry, removal from a class, suspension or termination under section 24.
24. Suspension or termination by FORME
FORME may suspend or terminate membership for serious or repeated breaches of these Terms or Studio Rules, non-payment, fraudulent or unauthorised use, unsafe behaviour, abuse, harassment or discrimination, deliberate damage to premises or equipment, or conduct presenting a material risk to others.
Except in urgent or serious cases, FORME will normally explain the concern, give the member a reasonable opportunity to respond, issue a warning where appropriate and confirm any suspension or termination in writing.
Where FORME terminates because of a serious member breach, it may retain sums properly due for services already supplied and may seek reasonable losses caused by the breach where legally recoverable. FORME will not automatically retain all future membership fees where no future service will be provided.
25. Personal belongings and lost property
Members are responsible for their personal belongings. FORME is not responsible for loss, theft or damage unless caused by FORME’s negligence or another legal responsibility that cannot be excluded. Valuable items should not be left unattended. Property left at the studio may be retained for 30 days before being donated, recycled or disposed of where reasonable attempts to identify the owner have failed.
26. Liability
FORME will take reasonable care in providing its services, maintaining equipment and operating the studio. Members remain responsible for providing accurate health information, following reasonable instructions, using equipment appropriately, exercising within their abilities and taking reasonable care of themselves and others. To the fullest extent permitted by law, FORME will not be responsible for loss or injury to the extent caused by a member’s failure to disclose relevant health information, disregard of reasonable safety instructions, unauthorised or reckless use of equipment, participation beyond the member’s known capabilities, or events that FORME could not reasonably have prevented. Nothing in these Terms excludes or limits liability for death or personal injury caused by FORME’s negligence, fraud, fraudulent misrepresentation, or any other liability that cannot legally be excluded.
27. Damage to studio property
A member may be charged the reasonable cost of repairing or replacing equipment or property that they deliberately damage or damage through reckless or clearly improper use. Members will not be charged for ordinary wear and tear, defects in equipment, or accidental damage that was not caused by a failure to take reasonable care.
28. Privacy and communications
FORME will process personal information in accordance with its Privacy Notice. Health information will be treated as special-category personal data and accessed only where reasonably necessary for safety, administration, legal obligations or another lawful purpose described in the Privacy Notice. FORME may send essential service communications concerning payments, bookings, waiting lists, membership renewal, timetable changes, closures, safety matters, cancellations and changes to these Terms. Marketing messages will be sent only where permitted by law. A member may opt out of marketing without losing essential service communications.
29. Photography and recording
FORME will not use identifiable images or recordings of a member for marketing without appropriate permission. Where a class is being photographed or recorded, members will be informed in advance and given a reasonable opportunity not to be included. A member’s refusal to appear in marketing content will not affect their membership or access to services.
30. Changes to these Terms
FORME may update these Terms to reflect legal or regulatory changes, booking-system changes, improvements in clarity or reasonable operational developments.
Material changes will be notified in advance. Changes will not be used to remove benefits already paid for or impose a significant disadvantage without providing an appropriate remedy or right to cancel where required.
31. Complaints
Members should raise concerns promptly so FORME has the opportunity to investigate and resolve them. Complaints should be sent to hello@formereformer.co.uk.
FORME will aim to:
- acknowledge a complaint within 3 working days;
- investigate it fairly;
- provide a substantive response within 7 working days where reasonably practicable; and
- explain any further escalation route where the matter remains unresolved.
32. Events outside reasonable control
FORME will not be responsible for delay or failure caused by circumstances it could not reasonably control, including severe weather, fire or flood, utility or internet failure, government restrictions, building access restrictions, emergency repairs, serious staff illness or events affecting public safety.
This does not allow FORME to retain payment indefinitely where no service is supplied. Where the disruption materially affects the purchased service, FORME will consider an appropriate extension, account credit, replacement service or proportionate refund according to the circumstances.
33. Transfer of the business or contract
FORME may transfer its rights and obligations under these Terms to another business operator, for example as part of a sale or reorganisation of the studio, provided that doing so does not materially reduce the member’s rights. The member will be notified of any transfer that materially affects them.
A member may not transfer their membership or obligations to another person without FORME’s prior written agreement, except where a product is expressly sold as transferable.
34. General legal terms
- If one term is found to be invalid or unenforceable, the remaining terms will continue to apply.
- A delay by FORME in enforcing a term does not mean that the term has been waived.
- No person other than the member and FORME has the right to enforce this agreement, except where the law provides otherwise.
- These Terms are governed by the law of England and Wales. Members retain the right to bring proceedings in any UK court available to them under applicable consumer law.
- These Terms do not affect the member’s statutory rights.
